This section provides resources to physicians or advanced practice providers seeking to better understand Goals of Care conversations with patients and various aspects of hospice and palliative care. 

Goals of Care Conversations Education Series
This series consists of three modules that will assist physicians and other health care providers in having effective goals of care conversations with patients using the REMAP communication framework. The modules should be completed in the order listed below as the content builds and progresses with each module.

Reframing and Responding to Emotion (Module 1)
This is the first of three modules that teaches physicians and other health care providers how to conduct goals of care conversations with seriously ill patients using the REMAP framework (Reframe, Emotion, Map Values, Align with Values, Propose a Plan). This course focuses on reframing (discussing prognosis and the big picture) and responding to emotion.

Mapping Values (Module 2)
This is the second of three courses that teaches physicians and other health care providers how to conduct goals of care conversations with seriously ill patients using the REMAP framework (Reframe, Emotion, Map Values, Align with Values, Propose a Plan). This course focuses on questions we can use to understand a patient’s values.

Aligning and Proposing a Plan (Module 3)
This is the third of three courses that teaches physicians and other health care providers how to conduct goals of care conversations with seriously ill patients using the REMAP framework (Reframe, Emotion, Map Values, Align with Values, Propose a Plan). This course focuses on aligning with a patient’s values and proposing a plan.

Hospice Concepts for the Generalist

This module will introduce the physician to the necessary steps to create a smooth transition to home hospice care for patients and families in the context of case studies. In the end, physicians will be better equipped to provide a better experience for the patients and families involved.

Substance Use Disorder in Hospice Patients

This course is designed as an introduction to caring for patients with substance use disorder (SUD) for hospice interdisciplinary teams. Strategies that can be used for treatment of SUD and pain management for patients with SUDs are presented in the context of a patient case study.
